- Delayed rescue response: Rescuers were slow to arrive because the nearby ship California had its radio turned off and didn't hear Titanic's emergency signals.
- Overconfidence in the ship's design: The makers of the Titanic thought it would "unsink," which made them not take enough safety measures.
